Fill out the Canada Visa Application
on your computer and click the blue "Validate" button. The box on the top of the application
will then state "Yes", or the form has not been filled out properly.

Only fill out Sections A and D on the Canada Authorization Form.
(We will complete the rest as your representative.)

Non-U.S. passport holders must send a copy of your green card or valid U.S. visa with valid I-94 card. |
