To expedite your New Passport, we will help you get started in the right direction. Don't be overwhelmed. We are here to assist you each step of the way.
First, you will need to gather all of the required information (listed below) and then take them to the nearest
Acceptance Agent to seal your documents and then send your package to us via Federal Express. You will
need to complete one government form called the DS-11 as well as the
LOA Form (which stands for Letter Of Authorization... it authorizes us to process your passport paperwork).
Then download and complete the Universal Passports & Visas Order Form with your payment information and fax it to: (214) 739-3414.

| Complete, download & print the DS-11 Form from the government website. |

Click "Continue" beneath the option that reads "Apply for a Passport Book". NOTE: Do NOT sign the application form until a Passport Acceptance Agent instructs you to do so. | Complete our Letter Of Authorization. |
The Letter Of Authorization simply enables us to act in the capacity necessary to resolve and answer any problems that may arise, to submit your passport application and to pick up your passport (and any related documents) when it has been issued by the Passport Agency. Get LOA here. * IMPORTANT: Must print and complete 2 copies of the LOA. |

| We need 2 passport photographs. |
|
Must be recent, identical and 2x2 inches. |
|
| We need a proof of your U.S. citizenship. |
This must be one of the following:
- An original certified copy of U.S. Birth Certificate
- A previous valid U.S. passport
- Report of Birth Abroad (Form FS-240)
- Certification of Birth Abroad (Form DS-1350), or
- Certificate of Citizenship or Naturalization from USCIS
NOTE: A certified birth certificate has a registrar’s raised, embossed, impressed or
multicolored seal, registrar’s signature and the date the certificate was filed with the
registrar’s office. The date must be within 1 year of your birth. Please note that some short
(abstract) versions of birth certificates may not be acceptable for passport purposes.

We need ONE form of primary identification.
|
This must be either:
- A previously issued & undamaged U.S. passport
- A Naturalization Certificate
- A Valid Driver’s License
- A Current Government ID (city, state or federal), or
- A Current Military ID (for military applicants and dependents).

| We need a copy of your travel itinerary. |
|
A copy of either your airline ticket(s) or flight itinerary will serve as the required proof regarding your departure date. |
|
| We need your $170 check made payable to the U.S. Dept. of State, as well as a $25 execution fee to the Acceptance Agent. |
|
These are government fees. Your payment to Universal Passports & Visas is for the expediting of your passport processing, but the Federal Government still requires their standard fees for a New Adult Passport. NOTE: Please indicate your Date of Birth in the memo field of your check. |

Download and complete the Universal Passports & Visas Order Form with your payment information and fax it to: (214) 739-3414. Payment is $250.00 for 24 hours, $200.00 for 2-3 days, $150.00 for 4-5 days, $100.00 for 6-10 days, or $60.00 for 11-15 days. We accept American Express, Visa, MasterCard, Discover or payment by check.

| Take all of these documents to your nearest Acceptance Agent. |

While at the Agent, inform them that you will be using a courier service (that’s us!). The Acceptance Agent will stamp the envelope before you send it to us. Do not open the envelope after the Agent seals your document. |
