Instructions for a Second Passport

To acquire a Second Passport, we'd like to help you get started in the right direction. Don't be overwhelmed. We are here to assist you each step of the way.

First, you will need to complete one government form (called the
DS-82), as well as what we call an LOA (stands for Letter Of Authorization... it authorizes us to process your passport paperwork).

Your Package Checklist:

1. Complete the Universal Passport Order Form. Download it here. To determine our fees, click here for our payment calculator.

2. Complete, download & sign the DS-82 Form from the government website. Here is the link: http://travel.state.gov/passport/forms/ds82/ds82_843.html.

3. Complete our LOA (Letter Of Authorization). NOTE: Leave the second blank empty -- it’s where you are authorizing us to handle the submission and pick-up of your passport documents (after they’ve been issued by the passport agency). Get LOA here. * IMPORTANT: Must print and complete 2 copies of the LOA.

4. We need a Statement requesting a Second Limited Passport. Be sure to sign. See our sample here.

5. We need your most recent passport. Needs at least two years of validity remaining. Passport must be signed. VERY IMPORTANT: Affix a note to your picture page stating "Do not cancel this passport."

6. We need 2 passport photographs. Must be recent, identical and 2x2 inches.

7. We need a copy of your travel itinerary. A copy of either your airline ticket(s) or flight itinerary will serve as the required proof regarding your departure date. NOTE: Itineraries must show conflicting trips (which shows need for a Second Passport).

8. We need your $135 check made payable to the U.S. Dept. of State. This is the government fee. Your payment to us is for expediting of the processing of your paperwork, but the Federal Government will still require its standard fees for a Second Passport. NOTE: Please indicate your Date of Birth in the memo field of your check.

9. We need your payment to us. You can do this one of two ways:

Now, if you are certain that you have followed and completed all of the above steps, gather everything together and head to your nearest Fed-Ex drop off location.
